The benefits of breastfeeding for mother and infant are widely documented. Despite these known benefits, national breastfeeding rates remain below the national health objectives of Healthy People 2010 — at least 75 percent of mothers who breastfeed their babies in the early post-partum period and 50 percent of mothers who continue breastfeeding until their babies are 5 to 6 months of age. Information from facilities providing maternity services and care for newborns was collected using a questionnaire adapted from the WHO/UNICEF Checklist for Evaluating the Adequacy of Support for Breast-Feeding in Maternity Hospitals. Wards, and Clinics. Data were collected on breastfeeding policies, staff mining, health education, discharge procedures, and delivering mothers demographics. Key participants in this study were lactation consultants or head nurses in obstetrics departments of hospitals in the metropolitan New Orleans area. The percentage of low-income new mothers ranged from 6-9 percent with an average of 47 percent. The percentage of women who initiated breastfeeding after delivery ranged from 20-75 percent, with only one facility meeting the Healthy People 2010 objective. The other hospitals rates were 60 percent or less. The majority of facilities had explicit policies for promoting and supporting breastfeeding. Staff training and support appeared to be sufficient. Almost all facilities provided specialized mining in lactation to staff members, with most facilities having at least one designated staff member available full-time to advise breastfeeding mothers. Implementation of existing policy appeared to be inconsistent. When there were set rules of allowing or not allowing the baby to stay in the room with the mother, respondents said “we do it anyhow”. Another example of inconsistent policy implementation was that despite most facilities having a policy to restrict an infants feeding before the initiation of breastfeeding, staff interpretation of this policy at three different facilities resulted in food and drink being given to newborns. Only five respondents reported that data are collected on the prevalence of breastfeeding initiation and breastfeeding at the time of discharge. Six facilities had a specific policy to give breastfeeding mothers special discharge packs containing breast pads, water bottles, and literature related to breastfeeding. These findings suggest that hospital facilities need to evaluate existing breastfeeding policies for consistency of interpretation and implementation. Furthermore, if no policy exists, one needs to be created. The impact of factors other than policy can be demonstrated in evaluating the one hospital that achieved the Healthy People 2010 objective. In addition to a low percentage of low-income status women delivering, this facility had patients with a high level of participation in prenatal care and a long-standing lactation consultant who was a strong advocate for breastfeeding in the facility and the community at large.

Part I of this series illustrated that the Department of Defense and its health care system, TRICARE, may not be meeting that goal, particularly after active duty mothers return to work. This study outlines the resources that would be required to optimally support breastfeeding after active duty mothers return to work. Selected policy implications of supporting breastfeeding are discussed, and recommendations for policy on breastfeeding are presented.The ability to successfully continue breastfeeding after returning to work involves four elements: space, time, support, and gatekeepers.1,2 The ideal situation is to have onsite daycare so that mothers can breastfeed their infants directly while they are at work. Since this is not possible in most cases, providing designated space in the workplace where mothers may express breast milk is critical because many active duty mothers who wish to continue breastfeeding after they return to work do not have private offices. The optimal space and equipment requirements are listed in Table I.The Oregon health division estimated the one time cost to provide a furnished, private room with a lock and an electric outlet as $585. For a room with an employer-owned hospital grade electric breast pump and written resource materials, the price was estimated as $1660.3 Although this might seem expensive, it is important to consider that more than 20 women can use a nursing mothers room during an 8-hour period.If space is at such a premium that a designated room cannot be provided, allowing the use of empty conference rooms or offices may suffice. However, women should not be forced to pump or breastfeed in bathrooms or closets. Just as you would not expect an employee to prepare or eat a meal in the bathroom, you should not expect her to provide nourishment for her baby in that environment.

Future Steel Vehicle will develop steel auto body concepts that address
alternative powertrains, such as advanced hybrid, electric, and fuel cell
systems. The goal of the research is the demonstration of safe, lightweight
steel bodies for future vehicles that reduce GHG emissions over the entire
life cycle.
At present, much effort is focused on reducing tailpipe emissions during
the driving life of vehicles. “From a total vehicle cradle-to-grave life
cycle perspective, steel is the most effective material for reduced
greenhouse gas emissions,” said Ian Christmas, International Iron and Steel
Institutes secretary general. “With this project, we will develop concepts
that should help automakers reduce GHG emissions over the entire vehicle
life.”
Future Steel Vehicle, a multimillion-dollar multiyear program, will consist

ABSTRACT Keywords: implantable cardioverter defibrillator, defibrillation threshold testing, cerebral ischemia, neurologic outcome The implantable cardioverter defibrillator (ICD) has become a standard therapy for a variety of patient groups (1). The assessment of defibrillation (DFT) efficacy at the time of implantation has long been the standard and required procedure (2, 3). Documentation of DFT efficacy provides the systems ability to sense, detect, and defibrillate ventricular fibrillation (VF). Different protocols are available for DFT testing (2, 4-6). Traditionally, a safety margin of at least 10 J between the maxi mum output of the pulse generator and the energy needed for defibrillation has been used because early studies indicate that lower safety margins were associated with high rates of failed defibrillation and sudden cardiac death (7,8). Subsequently, some investigators showed that monophasic defibrillation thresholds can increase over time with transvenous lead systems (9, 10). In selected patient, this increase required reoperation. Improvements in ICD technology permitted the routine active pectoral implantation of devices. Such active pectoral pulse generators, in combination with biphasic waveforms, result in marked reductions in DFTs and more stable thresholds long term (9, 11). The Low Energy Safety Study (LESS) examined whether the 10-J safety margin still was necessary using pectoral defibrillators with active can, biphasic shock waveforms (12). The main results of the study showed that a 4 to 6 J of safety margin above the DFT ++ is adequate for safe implantation of modern ICD systems.

Admixtures: Water resisting - not proofing? I note that there are several articles relating to concrete in the ground in CONCRETE, February 2004. These articles contain claims and statements relating to concrete performance in resisting water penetration. These range from “Waterproofing. can be achieved. by using a water resisting admixture within the concrete.” to “admixtures may be used that provides waterproofing..of concrete” to “watertight concrete”. These article give the impression that concrete is, or can be, made waterproof or watertight with the addition of an admixture. These terms are considered in Concrete Society Technical Report 18 (second edition) A guide to the selection of admixtures for concrete and CIRIA reports 139 and 140 Water-resisting basements. Both documents indicate that concrete cannot be waterproof or watertight in the true meaning of the words since this implies impervious or absolute prevention of penetration by water in liquid and or gaseous form. The correct term that should be used is water-resisting, describing the ability to resist water. This depends on the concrete quality and the effect of any integral admixture. The main function of water-resisting admixtures is to reduce surface absorption and/or passage of water through the hardened concrete. Other benefits such as improved durability may also result from their addition. It is worth noting that BS 8007: 1987 Design of concrete structures for retaining aqueous liquids only requires a certain concrete grade, with limitations on cement content and water/cement ratio to give the necessary water-resisting performance. Water-resisting admixtures are not required. The weak points in any water-retaining or resisting concrete structure are seldom the concrete itself, but cracking and performance of construction and movement joints. These can be controlled with careful design and construction, thereby limiting and subsequent ingress or loss of water.
Nearly all bridge decks are constructed of concrete. And almost all concrete bridge decks contain cracks–lots of cracks. Cracks are the bane of deck longevity. They allow the ingress of salts that cause corrosion of the reinforcing steel, exacerbating concrete cracking and loss of structural capacity. Why does concrete crack? Because as it dries it shrinks, and the shrinkage results in tensile stresses in the concrete, which generally exceed the tensile strength of the concrete as it is setting. Bridge decks are structural elements, hence reinforced. As the concrete dries and shrinks, the shrinkage is restrained by the reinforcing steel inducing compressive forces in the steel; compression in the steel creates tension in the concrete. The tensile strength of concrete is relatively low and takes time and maturity to develop; at early ages the tensile strength is negligible. Attempts to increase concretes tensile strength by increasing the cement content or by adding supplementary cementitious materials, such as silica fume, fail to improve the situation (or may even exacerbate it) because the additional water needed for the supplementary cementitious material increases shrinkage. For portland cement, a water-cement ratio of 0.22 to 0.25 is necessary to hydrate the cement–but at this water content the concrete is unworkable. So most concrete is placed at a w/c of 0.50 or more. The excess water results in concrete with a consistency that can be placed and finished, but the excess water isnt needed for cement hydration, and it therefore bleeds off to the surface, creating water channels. This increases total shrinkage since a significant volume of water is lost. The water channels formed during bleeding then provide a means of ingress for de-icing salts. The solution to this problem is to use shrinkage-compensating concrete, that is, concrete that contains a shrinkage-compensating cement, such as Type K, which causes the concrete to expand slightly. Since the reinforcing steel is bonded to the expanding concrete, it is placed in tension, which causes compression in the concrete. The compressive stress in the concrete negates the tensile stresses caused by shrinkage and prevents cracking. As the concrete dries and shrinks, the compressive stress is relieved, but by this time the concrete has developed some tensile strength that prevents cracking. Proper curing delays the shrinkage, allowing the concrete to develop greater tensile strength and therefore fewer cracks.
Q. Is there any research data about the levels of chlorides (salts) in soil that will cause damage to concrete and rebar? A. Chloride does not, in itself, damage concrete or metals embedded in concrete. It does make damage from cyclic freezing worse if the concrete is so exposed. The presence of any chloride in concrete has the potential to promote corrosion of embedded metal. The effect of chloride on concrete in soil will depend not only on the amount of chloride in the soil, but also on the permeability of the concrete. The extreme would be concrete exposed to seawater. If the soil in question is a former marine environment, then the presence of chloride is probably your least concern.
